Consultant Pharmacist The Consultant Pharmacist position is an external position for Polaris Pharmacy Services with the function of providing services to long-term care facilities.
Job Description: The consultant pharmacist's responsibilities include performing Medication Regimen Reviews (MRR), as required by regulation; providing written reports of these reviews, providing quarterly reports reflecting facility-level drug utilization and clinical management trends, recommendations to improve clinical care and potential savings using preferred drug management and formulary compliance, perform review of controlled substance utilization, reconciliation, and documentation; review medications stored in facility Emergency Drug Kit which would include review for outdated medications, and evaluate appropriate utilization and documentation of non-pharmacological interventions, either instead of, or in conjunction with, appropriate medication therapy.
- Conduct an entrance conference upon arrival to the facility and an exit conference when the consultant pharmacist services are completed for that month with the Administrator and/or Director of Nursing
- Meet with the facility Medical Director and other key prescribers as warranted
- Assists the facility to address/resolve pharmacy-related concerns and issues that affect resident care, medical care, quality of life, and/or regulatory compliance
- Identify opportunities for appropriate therapeutic interchanges
- Attend quarterly facility Quality Assurance Committee meetings and provide written reports regarding the status of the facility's pharmaceutical services, therapeutic interchange cost savings report, and staff performance
- Perform iMRR on residents who are anticipated to stay in the facility for less than 30 days, or who experience an "acute" change of condition, as identified by facility staff
- Evaluate facility documentation related to behavior and adverse effect monitoring for medications
- Assist (by telephone if necessary) with issues or questions that arise either before or on the day of any survey of the facility
- Follow all applicable government regulations including HIPAA.
- Other duties as assigned; Job duties may vary depending on business needs
Qualifications Qualifications: (must be met to perform the job at the required level)- B.S. in Pharmacy or PharmD degree with additional knowledge and/or Certification in geriatrics
- Hold a current pharmacy consultant license in the State of Practice
- Excellent clinical, organization, interpersonal, and communication skills with a commitment to providing high-quality pharmaceutical care
- 2-3 years experience in clinical pharmacy practice with at least one year of long-term care pharmacy practice preferred
- Thorough knowledge and understanding of all applicable nursing home regulations and guidance
- Experience using Microsoft Word, Excel, PowerPoint, and Outlook

- May sit or stand seven (7) to ten (10) hours per day
- The employee is occasionally required to sit; climb or balance; and stoop, kneel, bend, crouch, walk, crawl intermittently

- May lift and/or move up to 50 pounds

HOLIDAY & PTO POLICY We provide qualifying full-time employees with 23 PTO (Paid Time Off) days annually. PTO is used for holidays, vacation, sick leave, and personal leave.
